W-4 EMPLOYEE'S WITHHOLDING ALLOWANCE CERTIFICATE FORM (PDF 224 KB)
This must be completed prior to beginning work study employment. Complete this form so that the correct federal income tax can be withheld.
I-9 EMPLOYMENT ELIGIBILITY VERIFICATION FORM (PDF 444 KB)
This must be completed prior to beginning work study employment. Additional documentation must be provided to establish identity and employment authorization.
FEDERAL WORK STUDY PROGRAM PAYROLL AUTHORIZATION FORM (PDF 36 KB)
This must be completed prior to beginning work study employment. This form must be signed by student and supervisor.

Stafford Loan Processing Guide and Information
LOAN PROCESSING INFORMATION GUIDE (PDF 56 KB)
If you have signed your Award Notification Letter accepting a Stafford Loan, please read the information contained within this form carefully. Your financial aid cannot be processed until you have taken these very important steps. This form includes the directions to sign the Stafford Loan Master Promissory Note and the Entrance Counseling.
